Chaplain - Spiritual Care Coordinator

HCR ManorCare provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy. 1.Provides care for the pastoral and spiritual counseling needs of residents, families and staff. 2.Contacts patient/family clergy or spiritual representative when appropriate. 3.Conducts funeral services when requested by the patient’s family. 4.Provides bereavement follow-up services. 5.Participates in on call chaplain services. 6.Provides spiritual support to the hospice staff. 7.Submits paperwork in a timely manner to update and maintain patient medical records. 8.Prepares reports for the Patient Care Coordinator as required. 9.Participates in regularly scheduled interdisciplinary team meetings to coordinate the spiritual care of the patient/family, exchange information and problem solve, and receive staff support and education. 10.Responsible for development of the overall religious/pastoral program. 11.Coordinates an ongoing program of adult religious education and formation in spirituality. 12.Coordinates worship services per chaplaincy standards. 13.Attends meetings as required. 14.Participates in programs providing orientation, training and inservice. 15.Provides programs to the community on issues of loss. 16.Develops and leads inservice programs and other special programs for staff, residents, families and community. 17.Accepts other assignments deemed appropriate. 18.Assess and evaluate patients/families for spiritual needs. Educational Requirements

Graduate of accredited school of theology or Certification in Hospital or Pastoral Ministry.

Position Requirements

Experienced in providing an ecumenical approach in pastoral care. Demonstrated experience in working with patients and families coping with terminal illness and death. Excellent communication skills both verbal and written.

Position will be available May 21,2018.

Must have a Bachalor's degree from an accredited school of Theology plus 2 units of Clinical Pastoral Education or Masters in Social Work with 2 Units of Clinical Pastorl Education

Master's degree in Theological Studies.
