Heartland Hospice, a subsidiary of HCR ManorCare that provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy, is currently searching for a full-time Bereavement Specialist/Coordinator. This is a full-time day shift role with minimal on call after hour responsibilities.
As a Bereavement Coordinator you are responsible for developing, implementing and supervising the hospice bereavement program. You will provide bereavement support through grief support, memorial services, family assessments and conferences. In addition you are responsible for consultation to the interdisciplinary team, clients and families in issues related to grief and loss.
- Consults with the Volunteer Coordinator regarding assignment and supervision of the bereavement volunteers. Participates in orientation, training and evaluation of bereavement volunteers.
- Develops programs for bereavement group activities.
- Develops a resource library of written material, videos, and referral sources for families and the interdisciplinary team.

Bachelor's degree in counseling, theology or related field required; Master's degree in the area of mental health preferred.
Experience in caregiver support and counseling regarding issues of loss, grief, death and dying.
Experience in developing and conducting programs of support.
One year experience in home health care or hospice preferred.
Proficient computer skills preferred.
