Physical Therapist - Home Care - Full Time - Week Days
Heartland Home Health Care, a national, progressive Home Care provider has an immediate opportunity for a full-time Physical Therapist to support patients in their homes in the West Bend and surrounding areas. Our Physical Therapists participate in the development and implementation of the client's physical therapy plans of care and provide education to team members, clients and families in their homes. 100% travel necessary on a daily basis. At least one year or more work experience in physical therapy in a multidisciplinary setting is preferred.
Knowledge of principles, concepts, and methodology of physical therapy appropriate for terminally ill patients.
Excellent communication skills, and ability to develop and maintain rapport with clients and families.
Our main need is providing therapy to patients in the Washington County area!
Bachelor's or Master's degree in physical therapy from an accredited institution. Currently licensed as a Physical Therapist in the state and in good standing with the Board.
