Admissions Coordinator- Home Health & Hospice

We are currently searching for an experienced health care professional to provide service and support to our referral sources, community members and patients and families.
The Admission Coordinator is an internal customer service position that is responsible for coordinating the referral-to-admission process for home health and hospice patients. They provide administrative support to the admission team to ensure that all eligible patients receive care, barriers to admissions are addressed and resolved and agency(ies) growth and profitability is maximized.
