Admissions Coordinator- Home Health & Hospice - Less than Part Time
Heartland Home Health Care and Hospice, part of the HCR ManorCare family, a leading provider of home health care, hospice care, skilled nursing, memory care and post-acute care, is searching for a PRN or Casual (flexible as needed hours) to work weekday and weekend hours. Hours are flexible and on the weekends you could work from home.
The Admission Coordinator is an internal customer service position responsible to coordinate the referral-to-admission process for home health, hospice, and infusion patients. Provides administrative support to admission team to ensure that all eligible patients receive care, barriers to admissions are addressed and resolved and agency(ies) growth and profitability is maximized.

Associate Degree in related field required. Bachelor's degree preferred.
Demonstrated customer service experience Health care experience required. Travel is minimal but necessary on occasion as directed by sales manager.Job Specific Details:
