Demonstrates problem solving and decision making abilities. Ability to manage patient care with specific knowledge and experience in bedside nursing, symptom management, crisis intervention and family intervention. Demonstrated management ability with an interdisciplinary team and other health care providers. Qualifications

Minimum of one year experience in providing medical, oncology, home health care, or hospice nursing preferred, may be specified by state. Previous management or supervisory experience preferred. TRAVEL: Necessary on a daily basis, local travel 25-50%. Job Specific Details:

General Responsibilities
? Oversees admission process; conducts on-site initial patient assessment and periodic reassessment and
communicates with physician as needed.
? Plans, coordinates and delivers care to patient and families as appropriate.
? Delegates and assigns the patient/family to appropriate staff and team members, ensuring highest quality of care.
? Visits patients and families as needed.
? Supervises the staff responsible for clinical programs and provides assistance with development, implementation
and evaluation of the respective programs.
? Schedules, maintains and facilitates interdisciplinary team meetings for continual updating of patient care plans,
exchange of information and problem solving.
? Develops and promotes programs for continuing staff support, inservice and education.
? Implement and monitor compliance with rules and regulations of Federal and State regulatory agencies and
accreditation requirements, and policies & procedures of agency.
? Assures complete and accurate medical charts in compliance with all regulatory agencies.
? Interviews, hires and provides orientation and training for the staff.
? Provides staff performance evaluations/counseling/implementing appropriate disciplinary action of employees.
? Accepts other assignments deemed appropriate per Administrator.
? Collaborates with service vender regarding appropriate cost effective services and accurate billing.
? Directs branch quality improvement programs.
? Evaluates employees on-site as required per policy.
? Participates in on-call coverage.
? Monitors direct patient care costs and implements staff education to deliver cost effective, high quality and
appropriate care to patients/families.
? May serve on Professional Advisory Committee, Safety Committee, IOP Committee.
? Is knowledgeable of all Heartland services in order to foster referrals within the company.
? Able to perform functions of field staff. May be required to perform field assignments.
? Ensure growth & profitability of the company through the responsible use of company resources & educating the
community to our services.
? Willingly takes direction from Administrator/Director of Professional Services.
? Meets or exceeds established productivity standards.
? Adheres to established personnel policies.

Patient Rights
? Ensures that care is provided with respect for patient rights.
? Ensures protected health information is kept confidential
? Reports complaints made by patients or families to the appropriate persons
? Adheres to established personnel policies.
? Reports allegations of patient abuse, neglect and/or misappropriation of patient property.
Staff Development
? Educates staff in clinical aspects, as needed.
? Attends in-services and meetings as required.
? Participates in Circle of Care Training
Delivery of Care Responsibilities
? Participates in in-service training as requested by the office.
? Reviews and evaluates treatment plans for all patients served.
? Communicates appropriate clinical information to the interdisciplinary team regarding hospice care issues and
patient/family needs.
? Provides documentation of patient visits on the clinical record as appropriate.
? In conjunction with the physician and interdisciplinary team, assures appropriate treatment is given to patient and
families.
? Addresses patient care needs with families and significant others as appropriate.
Special Activities/Attributes
? Collaborates with service providers furnishing contract services to the patient.
? Participates in developing and updating policies and procedures as requested.
? Maintains confidentiality of necessary information.
? Thinks and acts calmly and logically to meet unusual occurrences of the job.
? Assures proper documentation for billing.
? Manages time efficiently and effectively.
? Interfaces with patients, families and staff to ensure customer satisfaction.
? Assures that a comprehensive discharge plan is completed on all patients.
? Communicates effectively with patients, families and other health care providers.
? Maintains clinical competency by assuming responsibility for acquiring knowledge and experience to meet
learning needs.

Education

Physical Demands
? Lifting 50 pounds maximum with frequent lifting and or carrying of objects weighing up to 25 pounds; Walking,
standing, talking, hearing, pushing, fingering, reaching, seeing, stooping, smelling, depth perception, color vision,
driving.
Travel
? Necessary on a daily basis, 50-75%. Occasional overnight travel may be necessary.

Working Conditions
? Client home/living facilities/office.
? Exposure to disagreeable conditions including odors, infection, illness, and physical contact from disruptive
patients.
? Must be flexible to accommodate completion of tasks, meet patient?s needs and referrals.

? Field staff, patients, families , physicians and community health care providers, office staff.
