Heartland Home Health Care and Hospice is part of the HCR ManorCare family, a leading provider of home health care, hospice care, skilled nursing, memory care and post-acute care.
FT Triage Nurse. The position would be a fulltime salaried position working Thursday 5PM to 8:30AM and Friday 5PM to Monday 8:30AM. The RN would triage calls, work the ladders in Home Care/Home Base to make sure visits are ready for the staff devices. HCHB experience preferred but not required.

Currently licensed as a Registered Nurse (RN) in the state and in good standing with the Board in which he/she will practice.
A minimum of six months nursing experience within the past three to five years.
100% travel on a daily basis.
